Referring dentists are welcome to refer patients to our office. We accept referrals for complex cosmetic dentistry, TMJ treatment, and IV sedation dentistry. Once the form has been completed, please have the patient contact us for an appointment and bring the referral slip with them.
If you would like us to call you before treatment, please let us know. We are happy to perform comprehensive care, or treat for a specific referral and refer back to you for comprehensive care. We promise to always communicate and coordinate treatment efficiently for referring providers.
