Does Botox help TMJ, and how long does it take to work?

Masseter Botox for TMJ takes time to work. Most patients notice the greatest muscle relaxation between days 5 and 21, with effects gradually wearing off around months 3 to 4.

Days 0 to 3: Your Jaw Is Adjusting

During the first few days, you may feel tightness, tenderness, or an unfamiliar sensation in your jaw. Some patients actually feel more tender than before their injections.

The masseter muscle has not fully relaxed yet. These early sensations are usually no cause for alarm.

Days 5 to 21: Muscle Relaxation Peaks

Between the first and third week, Botox reaches its peak effect. You may notice:

  • Less jaw tension and clenching
  • Reduced teeth grinding
  • Relief from associated headaches

This is when I can get a clearer picture of how well your jaw muscles are responding.

Months 3 to 4: Muscle Function Gradually Returns

Botox for TMJ is not permanent. As its effects wear off, muscle function gradually returns.

For most patients, this is when we discuss a maintenance appointment to help sustain their relief.

Why does my jaw look more square or bulgy after Masseter Botox?

A temporary jaw bulge after Botox can happen when the main part of your masseter muscle relaxes but smaller surrounding muscle fibers remain active. Those fibers may contract more noticeably, creating a bulge or uneven appearance that usually settles as the treatment takes full effect.

Botox changes how your masseter muscle functions, so it can also subtly change your jawline. As the dominant muscle reduces in size, you may notice contours you had not paid attention to before.

A change in your jaw’s appearance does not automatically mean something went wrong. In most cases, these changes are part of the muscle’s response to treatment.

Is it normal for my jaw to feel worse during the first week?

Yes, some patients temporarily feel worse during the first week after TMJ Botox. Different parts of the muscle can relax at different rates, creating a short period of uneven muscle activity.

Your jaw may feel a little uncoordinated while those muscles adjust. That does not necessarily mean the treatment is failing.

Should I wait or get more Botox if my jaw still feels tight?

I recommend waiting at least two full weeks before deciding whether you need a Botox top-up. The treatment needs time to reach its full effect, and many early concerns about unevenness or insufficient relief resolve without additional injections.

If your jaw still feels tight after that period, we can assess the result and decide whether an adjustment makes sense.

When would I need Botox in my temporalis muscles too?

If you have significant temple headaches or persistent clenching, I may recommend Botox injections in the temporalis muscles at a follow-up appointment. Treating these muscles along with the masseter can provide more comprehensive relaxation across the jaw muscle system.

Not everyone needs additional injections. I base that decision on your symptoms and how you respond to the initial treatment.

What if Botox does not relieve my TMJ pain?

If your discomfort continues after a properly dosed Botox treatment, the problem may involve more than muscle tension. An underlying joint condition, such as disc displacement or arthritis, could be contributing to your symptoms.

In those cases, I may recommend:

  • Advanced imaging: To investigate a possible structural problem in the jaw joint.
  • A custom orthotic splint: To address the structural component of your TMJ disorder.

Botox can help relax overactive muscles, but it is not always the complete answer when the joint itself is involved.
